Testing Instructions
Are you testing at a different rink/club?
You will need to submit a Permission to Test Form.
- Download and complete the form.
- Email the completed form to our Membership Chairperson for their signature.
- Submit the completed form at least 7 days before your scheduled testing date.
- The Membership Chair will verify that you are a member in good standing for the current membership year, sign the form, and send it to the hosting club.

Test requirements & guidelines
Video Requirements: It’s important to follow all steps to ensure your test will be eligible to judge.
Videos must be taken with 14 days of submission date.
Affidavit form
Performance affidavit: Affidavit forms are required to be completed during the test with
the Skater, Coach, Videographer, and Proctor Information, Signature, and Certification.
