Are you testing at a different rink/club?
You will need to submit a Permission to Test form.
1. Download & complete the form.
2. Email the completed form to our Membership Chairperson for their signature.
3. The Membership Chair will sign-off on your form and send it to the hosting club.

Skaters, please confer with your coaches about video submission requirements.
We ask that all video submissions be put in Google Drive with the file titled with the skater’s name and test.

Video Requirements:
It’s important to follow all steps to ensure your test will be eligible to judge.
Videos must be taken with 14 days of submission date.
Performance affidavit
Affidavit forms are required to be completed during the test with
the Skater, Coach, Videographer, and Proctor Information, Signature, and Certification.
